Element Description
ActualOrPresumedDateOfDeath

Actual or presumed date of death. Corresponds to item 29 of the U.S. Standard Certificate of Death.

AddressLine

Part of an address that contains the house number, apartment number, street name, street direction, P.O. Box number, delivery hints, and similar address information. (Source: HL7 FHIR).

Age

The decedent's age in years at last birthday. Corresponds to item 4a of the U.S. Standard Certificate of Death.

AutopsyPerformed

Whether an autopsy was performed (true) or not (false). Corresponds to item 33 of the U.S. Standard Certificate of Death.

AutopsyResultsAvailable

Were autopsy findings available to complete the cause of death? Corresponds to item 34 of the U.S. Standard Certificate of Death.

Birthplace

Decedents place of birth. Corresponds to item 6 of the U.S. Standard Certificate of Death.

CauseOfDeathCondition

A condition that resulted in the underlying cause of death. Corresponds to part 1 of item 32 of the U.S. Standard Certificate of Death. Other significant conditions that contributed to death but did not lead to the underlying cause of death should be captured using a ContributedToDeathCondition.

CauseOfDeathSection

A section containing observations that detail the cause of death and associated information.

Certifier

The certifier of the death record. Corresponds to items 45, 46, 47, 48 and 49 of the U.S. Standard Certificate of Death.

CertifierType

A code that represents the type of death record certifier. Corresponds to item 45 of the U.S. Standard Certificate of Death.

City

The name of a municipality, city, town, village or other community or delivery center. (Source: HL7 FHIR).

CodeableConcept

A set of codes drawn from different coding systems, representing the same concept.

CodeSystem

A formal terminology system.

CodeSystemVersion

The version of the vocabulary being used, if applicable.

Coding

Coding of a concept, drawn from a controlled vocabulary. Includes the vocabulary and version, if applicable. May include a display text, and a descriptor expressing the intended interpretation of the code.

Condition

A disorder, abnormality, problem, injury, complaint, risk, functionality, concern, illness, disease, ailment, sickness, affliction, upset, difficulty, disorder, symptom, worry, or trouble. A ConditionPresent finding implies the existence of a condition as an entity. 'Condition' is interpreted broadly to cover states of existence - things the patient is or has.

ContributedToDeathCondition

A significant condition that contributed to death but did not result in the underlying cause captured by a CauseOfDeathCondition. Corresponds to part 2 of item 32 of the U.S. Standard Certificate of Death.

Country

Country - a nation as commonly understood or generally accepted, expressed in ISO 3166 Alpha-2 (2-letter) codes.

DateOfBirth

Decedents date of birth. Corresponds to item 5 of the U.S. Standard Certificate of Death.

DatePronouncedDead

Date pronounced dead. Corresponds to items 24 and 25 of the U.S. Standard Certificate of Death.

DeathFromTransportInjury

Injury leading to death associated with transportation event. Corresponds to item 44 of the U.S. Standard Certificate of Death.

DeathFromWorkInjury

Did death result from injury at work? Corresponds to item 41 of the U.S. Standard Certificate of Death.

DeathRecord

A document that details information about the death of an individual. The contents correspond to items included in the U.S. Standard Certificate of Death.

DeathRecordContents

Defines the contents of a death record document. The content is mapped from the U.S. Standard Certificate of Death.

Decedent

The subject of the death record (the decedent). Extensions are used to capture additional information from the U.S. Standard Certificate of Death where the Patient resource does not include an equivalent property.

DecedentID

An identifier for the decedent.

DecedentRelative

A relative of the decedent. Used as a basis for surviving spouse, father and mother profiles.

DisplayText

A string meant for reading by a person, for example, accompanying a code.

Disposition

Disposition of the decedent's body. Corresponds to items 18, 19, 20, 21, 22 and 23 of the U.S. Standard Certificate of Death.

DispositionFacility

The place of disposition of the decedent's remains. Corresponds to items 19 and 20 of the U.S. Standard Certificate of Death.

DispositionType

The method of disposition of the decedent's remains. Corresponds to item 18 of the U.S. Standard Certificate of Death.

District

The name of the administrative area at a level below that of a state but above that of a city or town. In the US, a county. Outside the US, a district or the equivalent. (Source: HL7 FHIR).

Education

Decedent's level of education. Corresponds to item 51 of the U.S. Standard Certificate of Death.

Entry

Metadata attributes that apply to any item represented in the standard health record. An Entry may not belong exclusively to a single person's health record, but could represent an entity that surfaces in multiple records, such as organizations or practitioners. If the entry belongs to a single person's record, then the identity of the person of record must be recorded.

EntryId

A persistent, permanent identifier for an entry in a health record, unique within the scope of the health record.

EntryType

SHR data element identifier, as a URI. The EntryType array includes the entire 'based on' hierarchy to enable searching for classes of things, for example, searching for all vital signs, or all types of behavioral observations.

Ethnicity

Decedent of hispanic origin? Corresponds to item 52 of the U.S. Standard Certificate of Death.

FacilityName

The name of a facility or institution

Father

The father of the decedent. Corresponds to item 11 of the U.S. Standard Certificate of Death.

FuneralDirector

Funeral Director. Corresponds to items 22 and 23 of the U.S. Standard Certificate of Death.

FuneralFacility

Name and address of the funeral facility. Corresponds to item 21 of the U.S. Standard Certificate of Death.

IDType

The type of identifier for a decedent

Industry

Kind of industry or business in which the decedent worked. Corresponds to item 55 of the U.S. Standard Certificate of Death.

Informant

The decedent's informant. Corresponds to item 13 of the U.S. Standard Certificate of Death.

InjuryDateTime

Date and time of injury

InsideCityLimits

Whether the address is within city limits (true) or not (false).

Job

Type of work done during most of decedent's working life. Corresponds to item 54 of the U.S. Standard Certificate of Death.

LegalName

The decedent's legal name plus any akas. Corresponds to item 1 of the U.S. Standard Certificate of Death.

MannerOfDeath

The manner of the decendents demise. Corresponds to item 37 of the U.S. Standard Certificate of Death.

MaritalStatusAtDeath

The marital status of the decedent at the time of death. Corresponds to item 9 of the U.S. Standard Certificate of Death.

MedicalExaminerContacted

Whether a medical examiner or coroner was contacted. Corresponds to item 31 of the U.S. Standard Certificate of Death.

Mother

The mother of the decedent. Corresponds to item 12 of the U.S. Standard Certificate of Death.

Observation

Base class for observations (measurements, tests, or questions) and a value containing the result (answer). The subject of a finding can be the entire patient, or an entity such as a location body structure, intervention, or condition. Things observed about the subject can include social and behavioral factors, subjective and objective observations, and assessments.

ObservationCode

A code that represents what the finding concerns, for example, the subject's height, blood pressure, disease status, wound dimensions, diabetes risk, etc. Although named ObservationCode, in different contexts the same attribute might be more naturally referred to as a property, observable, or test code. In assertions (observations without an explicit question), the observation code is defaulted to a value representing 'assertion'.

Occupation

Decedent's usual occupation. Corresponds to items 54 and 55 of the U.S. Standard Certificate of Death.

Onset

The beginning or first appearance of a mental or physical disorder.

Patient

A person in the role of a patient, including extended demographic information about the subject of this health record.

PlaceOfDeath

The location where death occurred. Corresponds to items 14, 15, 16 and 17 of the U.S. Standard Certificate of Death.

PlaceOfDeathType

The type of location at which death occurred.

PlaceOfInjury

Description of the place of injury, e.g. decedent's home, restaurant, wooded area

PostalAddress

An address expressed using postal conventions (as opposed to GPS or other location definition formats). This data type may be used to convey addresses for use in delivering mail as well as for visiting locations and which might not be valid for mail delivery. There are a variety of postal address formats defined around the world. (Source: HL7 FHIR).

PostalCode

A sequence of letters and digits used as part of a postal address, often designating a geographic region

Practitioner

A person in the role of a practitioner.

Race

Decedent's race. Corresponds to item 53 of the U.S. Standard Certificate of Death.

RelationshipType

A code that represents the type of relationship.

ServedInArmedForces

Whether the decedent ever served in the US armed forces. Corresponds to item 8 of the U.S. Standard Certificate of Death.

Sex

The birth sex of the decedent. Corresponds to item 2 of the U.S. Standard Certificate of Death.

SocialSecurityNumber

The decedent's social security number. Corresponds to item 3 of the U.S. Standard Certificate of Death.

State

Sub-unit of a country with limited sovereignty in a federally organized country. A code may be used if codes are in common use (i.e. US 2 letter state codes). (Source: HL7 FHIR).

Subject

The person or thing that this entry relates to, usually the Person of Record. However, not all entries have health information specifically about the patient, but in different contexts, could refer to a fetus, family member (living or dead), device, location, organization, finding, condition, or intervention.

SurvivingSpouse

The surviving spouse of the decedent. Corresponds to item 10 of the U.S. Standard Certificate of Death.

TimePeriod

A period of time defined by a start and end time, date, or year. If the start element is missing, the start of the period is not known. If the end element is missing, it means that the period is ongoing, or the start may be in the past, and the end date in the future, which means that period is expected/planned to end at the specified time. The end value includes any matching date/time. For example, the period 2011-05-23 to 2011-05-27 includes all the times from the start of the 23rd May through to the end of the 27th of May.

TimePeriodEnd

The time at which something is to end or did end. Boundary is considered inclusive.

TimePeriodStart

The time at which something is to take effect, start, or did start. Boundary is considered inclusive.

TimingOfRecentPregnancyInRelationToDeath

Timing of recent pregnancy in relation to death. Corresponds to item 36 of the U.S. Standard Certificate of Death.

TobaccoUseContributedToDeath

Did tobacco use contribute to death. Corresponds to item 35 of the U.S. Standard Certificate of Death.

TypeCode

A code that represents the type of a record.
